Social Worker/Case Coordinator

STATEMENT OF PURPOSE:
Provides direct case management and referral services to children and
families in the Early Intervention program and Children’s Development
Center.
SPECIFIC ACTIVITIES INCLUDE, BUT ARE NOT LIMITED TO:
• Assists Senior VP of Educational Services and other staff in completion of assessments;
• Formulates, implements and evaluates individual child needs in cooperation with other program staff and consultants;
• Provides referrals to Lighthouse programs, DHS and other appropriate community services;
• Provides crisis intervention and referrals to offer proactive intervention for families in need;
• Maintains records, write reports, and provide evaluations as required by programs and the agency;
• Provides counseling to families who need guidance, understanding, adjustment to their child’s loss of sight, and referral to resources that will maximize each individual’s goals for independence;
• Makes recommendations to Senior VP of Educational Services and supports implementation or refinement of additional services, such as support groups, as needs emerge;
• Collaborates with Senior VP of Educational Services to seek possible revenue sources, such as insurance billing and private contributions, for social work services;
• Maintains an active interest in the professional field at all levels by attending seminars and conferences to increase knowledge and maintain professional credentials, in addition to representing Lighthouse constituents and priorities on community-based committees, as applicable;
• Performs other duties as assigned by Supervisor.
QUALIFICATIONS:
• Master's degree in social work and LCSW required;
• State of Illinois Type 73 Certification required;
• Excellent organizational and communication skills needed;
• Minimum of two years work experience in a school setting
or other multi-disciplinary setting required, with concentration in working with children who are blind, visually impaired, and/or multi-disabled preferred;
